Frequently Asked Questions
If your question is not answered here, please contact your Care Coordinator.

My AHN patient needs imaging

  • Please fill out an AHN referral form including the CPT codes for the required tests. Fax the referral form to AHN at 775-770-3335 and we will process the referral. All imaging is done through Renown Health and Saint Mary’s.
  • If you need an AHN referral form – download one here or call 775-770-6035 to have one faxed to you.

My AHN patient needs lab work

  • Please fill out an AHN lab request form, keep one copy for yourself and give one copy to the patient. The patient can go to either Saint Mary’s or Renown Health outpatient labs. Directions to the labs are on the lab form.
  • Please DO NOT send any AHN patient to Lab Corps or Quest as we do not have contracts with those providers.
  • All pap smears must be sent to the Nevada State Health Laboratory. You have been given forms with your specific office information on which to write orders for pap smears. Please use these always for AHN members. Again, this is the only way our members will get their contracted discounts.
  • If you need an AHN lab form, send us an email or call 775-770-6035 to have one faxed to you.

What do I charge my AHN patient?

It depends on their tier. You can find the member’s tier number on their card. The member number begins with either T1 or T2, identifying the tier.

My AHN patient needs surgery

Please fill out an AHN referral form including the CPT codes, date of surgery, length of surgery, and if there will be an assist and fax it to the AHN office at 770-3335. The patient’s care coordinator will process the referral and inform the patient of provider fees due to all providers associated with the surgery.

Surgeries can be done at either Renown Health or Saint Mary’s.

The patient had an in-office procedure what do I charge them?

To ensure the success of AHN, our members need to be well informed about how much they need to pay. When scheduling appointments for AHN members, please tell them what you expect the charges to be.
  • Minor office procedures – AHN asks that any minor procedure be included in the office visit fee.
  • Major office procedures – if a provider wishes to perform a major procedure on an AHN member, please schedule the procedure for a subsequent office visit if the member is not prepared to pay at that time.
    • When scheduling the appointment, be sure to inform the member of the exact cost of the procedure. For your convenience, the appropriate percentage of Medicare allowable is printed on each AHN member ID card.
    • Medicare allowable rates can be found at www.cms.hhs.gov/pfslookup
